Charles Patton Hayes, Jr., died December 10, 2011. Chuck was born April 25, 1934 in Schenectady, New York, to Charles Patton Hayes, Sr. and Susan Elizabeth Hearn, both of Asheville, North Carolina. He grew up in Fort Wayne, Indiana. In 1950, he graduated from Marblehead High School in Marblehead, Massachusetts. Chuck attended Duke University and received his medical degree from there in 1959 under the tutelage of the renowned Chairman of the Department of Medicine, Eugene A. Stead. He completed a two year Residency in Internal Medicine in Galveston, Texas, followed by two years of specialty training in Nephrology back at Duke. He continued his training in Nephrology as a member of the Duke Medical School faculty for the next five years.

Chuck entered his practice at Riverside Clinic in Jacksonville, Florida in 1969, and pursued a medical career that spanned thirty years. He was a driving force behind the establishment of North Florida’s first free-standing kidney dialysis clinic and played a pivotal role in the creation of a renal care system and the ESRD (End-stage Renal Disease) Network in the state of Florida. He was a distinguished leader within the Jacksonville medical community and served in the following positions: Medical Director at Dialysis Clinics of Jacksonville, Inc.; President of the Jacksonville Community Council Inc.; President and CEO of Riverside Hospital; President of Riverside Clinic; President of the Duval County Medical Society; and Chief of Staff at Riverside Hospital.

In addition to his leadership roles in Jacksonville’s medical community, Chuck served in numerous leadership positions in the State of Florida medical community, including: Treasurer of the Florida Medical Association; Alternate Delegate from Florida to the American Medical Association; Chairman of the Board of Doctors Health Plan; President of the Florida Society of Internal Medicine; Member of the Board of Directors of Blue Cross/Blue Shield of Florida; Chairman of the Health Care Financing Council of FMA; Chairman of the Peer Review Committee of FMA; President of the Florida Society of Nephrology; Chairman of Kidney Advisory Council; Florida Division of Health; and Chairman of the Medical Advisory Board for Northeast Florida.

Chuck was a longtime communicant of St. Mark’s Episcopal Church, where he served on the vestry and enjoyed a men’s prayer group, and recently became a founding member and leader of Holy Trinity Anglican Church. He also served on the Advisory Board for the Healing Touch ministry. He was an avid golfer from an early age, who loved to play in various local tournaments and enjoyed teaching the game to his son and his grandchildren. He regularly enjoyed rounds of golf with friends and colleagues during his forty year membership in the Timuquana Country Club.

Chuck was a cherished husband, father, and grandfather, who is survived by his wife of fifty-two years, Jo Moore Smithwick Hayes, his daughter, Barrie Elizabeth Hayes, his son, Charles Gregory Hayes and wife Martha Matthews Hayes, two grandchildren, Matthew Gregory Hayes and Grace Ann Hayes, his sister, Margaret Sue Hayes Van Winkle and husband, Charles, his niece, Elisabeth Van Winkle Cox, her husband, John, and their children, Charlie, Christopher, and Andrew.

The family wishes to thank the many doctors and other caregivers, and special friend, Ernest Carraway, who ministered to Chuck during his long illness. We are grateful to our many dear friends for their love and prayers.

A memorial service will be held at 11:00 am Monday, December 19, 2011 at Holy Trinity Anglican Church, 3889 Eloise Street, Jacksonville, FL 32205. In lieu of flowers, the family requests that donations be made to a charity of choice.
